Understanding Support at Home
As you are aware, significant changes are coming to Australia’s aged care system. From 1 November 2025, the current Home Care Packages program will transition to the new Support at Home model .

That’s a great first step! During your assessment, a trained assessor will talk to you about your health, daily activities, and support needs. Afterward, you’ll receive a Support Plan outlining what services you’re eligible for. You can then choose a provider to begin services.

Mercy Health can provide short-term restorative care, increased clinical support, or a reassessment to adjust to your changed care needs. Contact our friendly Customer Enquiry Centre on 1300 478 776. Or if you are a current client, please contact your Care Partner. You can also contact My Aged Care on 1800 200 422 to request additional support.
Support at Home currently affects Home Care Package and Short-Term Restorative Care clients. CHSP will remain unchanged until at least July 2027. More information will be provided closer to the CHSP transition.
You’ll receive a Notice of Decision and a Support Plan from My Aged Care after your assessment. This outlines your approved services and funding level. You can also check your status by calling My Aged Care on 1800 200 422 or logging into your online account.

You may qualify for additional support if:
- Your health or mobility has declined
- You’ve had a fall or hospitalisation
- Your carer is no longer available
Contact My Aged Care on 1800 200 422 to request a reassessment. You may also be eligible for short-term restorative care or a higher classification.
If your father already receives a package, he’ll transition automatically on 1 Nov 2025 as a “transitioned HCP care participant” with no disadvantage in funding.

Under Support at Home, Registered Nurse (RN) services are charged based on the actual time spent, not automatically in one-hour blocks. These services are fully subsidised by the government, so there are no out-of-pocket costs for eligible clients unless you choose additional services beyond your plan.
A Level 3 classification supports people with intermediate care needs including personal care, domestic help, transport, nursing, allied health, and social support.
You can access non-urgent services like gardening and home care through the Commonwealth Home Support Programme (CHSP) or the new Support at Home program (if you’re transitioning from a Home Care Package). Start by contacting My Aged Care on 1800 200 422 to arrange an assessment. Once assessed, you’ll be matched with services based on your needs and eligibility.
