We know that navigating the path to getting extra care and support can feel challenging. To make things easier, we’ve outlined six simple steps to help you get started with accessing home care services. Our team is here to assist you at any stage along the way.

Step 2

Contact the NDIS and complete the application process

If you think you’re eligible for the NDIS, you can contact your local NDIS office on 1800 800 110 or an NDIS partner to apply.

Download and complete the NDIS Access Request Form which can be found on the NDIS website, and email it to [email protected] or post it to PO Box 700, Canberra, ACT 2601 with the required supporting information.

Once your application is approved, you will meet with an NDIS planner. They will work with you to identify the specific supports that will help you live independently.

Step 3

Determine the home care services you require

An NDIS planner will discuss your personal circumstances with you, the range of supports available to you and the goals you want to pursue.

Based on your discussion, a support plan will be developed.

Step 4

How to choose the right home care provider for you

Once your support plan is approved, you have the flexibility to choose a services provider that will deliver the services outlined in your plan.

The NDIS website has a helpful Provider Finder that you can use to find a registered provider.

Important things to consider when choosing a provider:

  • ensure the provider is a registered provider, insured, and complies with NDIS Practice Standards
  • ask for proof of security checks and qualifications
  • ensure the provider offers all the support services you require.

Book a home visit

We recommend that you contact us as soon as possible to commence your support services. A member of our team will contact you to arrange a home visit to assess your needs and preferences. Based on this, we’ll develop a personalised care plan tailored specifically for you. They will also talk through specifics step by step so you understand and are comfortable with each aspect of your care plan.
